Hospital to Home – A Collaborative Approach to Bereavement Support

This innovative, opt-out model delivers a stepped approach to care in partnership with NHS maternity and psychology teams, ensuring that everyone has access to emotional, psychological, and social support.

The session will showcase case studies and evaluation data to demonstrate how combining NHS clinical expertise with the flexible, person-centred ethos of Held in Our Hearts improves access and continuity in bereavement support whilst easing pressure on NHS services.
